The live recommendation record contains six open items

The Postal Service inspector general’s current recommendation portal lists six open recommendations from its Seattle mail-theft review. Management agreed with all six, and none carries a stated questioned-cost or funds-put-to-better-use value.

The governmentwide Oversight.gov record describes five recommendations. Comparing the two federal records exposes a one-item metadata mismatch; the USPS OIG’s live recommendation portal provides the more detailed current ledger.

The fixes concern keys, training and escalation

The open actions include key-inventory controls at Wedgwood and the Seattle Carrier Annex, required training and 24-hour accountability, reconciliation of duplicate or missing records, remediation reporting, district-manager visibility and guidance for documenting police incident numbers for lost or stolen keys.

These are control failures and proposed remedies. The recommendation list does not say that every missing key produced a stolen mailpiece or identify a dollar loss.

Scale makes small control failures consequential

Seattle-area facilities processed more than 1.3 billion mailpieces in fiscal 2025, according to the audit. That volume explains why key custody, timely escalation and consistent inventory records matter even when the audit does not assign a monetary value.

Completion should be measured by reconciled inventories and tested adherence, not by closing an item solely because a revised instruction was issued.

Method and limits

What’s the Scoop With Broach compared the USPS OIG audit page, its live open-recommendation portal and Oversight.gov’s metadata. The mismatch is a public-record comparison, not a claim that one agency concealed a recommendation.

The newsroom did not obtain investigative case files or identify theft victims. Open recommendations show unresolved audit actions, not proof that current operations remain unchanged at every facility.
